This form records your name, your district role, and your signature to confirm that you have read and agreed to the District's Non-Disclosure and Acceptable Use Agreement before you are granted access to District systems.

  • Everything you enter is sent over a secure, encrypted connection (HTTPS).
  • Your signed Agreement is stored inside Rotary District 9660's private Microsoft 365 environment, where data is encrypted and access-controlled.
  • It is seen only by the District IT team, who are responsible for administering system access.
  • The information is used solely to record your agreement and to administer your access. It is not sold, published, or shared for any other purpose.
  • It is held and used in accordance with applicable privacy legislation and Rotary's privacy policy.
