I, the undersigned parent/guardian, consent to my child named above being transported in the vehicle identified above, driven by the adult named above, for the purpose of travel to and/or from the RYPEN camp organised by Rotary District 9660.
I understand that this transport is provided as a courtesy by the driver named above, and that the camp will release my child only to that driver or to another authorised contact. Where the driver is a Rotary volunteer, Rotary District 9660 and its volunteers will take all reasonable care for the safety of my child during transport.
